- Patient Education and Support: Provide education and support to patients and their families, promoting health literacy and empowering them to manage their care.
- Care Coordination: Coordinate patient care with other healthcare professionals, ensuring seamless transitions and optimal outcomes.
- Medication Administration: Safely administer medications, adhering to organizational policies and procedures, and state regulations.
- Leadership and Mentorship: Serve as a subject matter expert and clinical mentor, providing guidance and support to colleagues.
- Licensure and Certification: Current Licensed Practical Nurse (LPN) licensure from the state of employment or LPN applicant.
- Education and Experience: 1 year of clinical patient care experience preferred.
- Skills and Abilities: Strong communication and interpersonal skills, ability to work effectively in a fast-paced environment, and proficiency in electronic health records (EHRs).
This role requires the ability to work in a dynamic, patient-centered environment, with periods of high stress and fluctuating workloads. You may be exposed to physical altercations, verbal abuse, and hazardous substances or body fluids. Universal precautions, personal protective equipment, and infection control policies will be followed.
Physical RequirementsThis role requires manual dexterity, ability to perform shift work, and maneuver the weight of patients. You will be required to hear alarms, telephone, and audio recordings, and reach above shoulder level. Repetitive arm and hand movements, finger dexterity, and color vision are also essential.
