Job SummaryThe Home Health Nurse Specialist provides comprehensive nursing care in the client's home, including health and psycho-social evaluation, monitoring of the client's condition, health promotion, and coordination of services. This role requires a strong understanding of nursing care processes, patient observation, and delivery of prescribed treatments. The Home Health Nurse Specialist will also conduct initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points.
ResponsibilitiesThe Home Health Nurse Specialist will be responsible for:
- Providing direct nursing care, including patient observation and delivery of prescribed treatments.
- Evaluating outcomes of patient care and identifying discharge planning needs.
- Maintaining patient records and charts to ensure accurate record of treatment.
- Providing instructions to patients/families for use of products and services utilizing nursing process to address learning needs.
- Preparing, administering, monitoring, and discontinuing IV fluids.
- Administering medications and treatments as prescribed by the physician.
- Performing phlebotomy, laboratory specimen collection, and point of care testing.
To be considered for this role, applicants must have:
- Graduate of an accredited nursing program.
- 2 years of RN experience with one (1) year of experience with adult IV expertise required.
- (RN) REGISTERED NURSE by Board of Nurse Examiners for the State of Texas (or BNE Compact State).
- Valid Texas driver's license, proof of driver's insurance.
- (BLS) BASIC LIFE SUPPORT accredited course by the American Heart Association (AHA) or American Red Cross (ARC).
$78,000 - $110,000 per year, depending on experience.
