Home Health Nurse

We are seeking a compassionate and skilled Home Health Licensed Vocational Nurse (LVN) to provide high-quality nursing care to patients in the comfort of their homes.
As a Home Health LVN, you will be responsible for assessing patients' health needs, administering medication, and educating patients and their families on managing their conditions.
Your role will be crucial in helping patients maintain their independence and improve their overall quality of life.
Responsibilities:
- Conduct thorough assessments of patients' health status and develop individualized care plans in consultation with the healthcare team.
- Administer medications, treatments, and wound care as prescribed by the physician.
- Monitor patients' vital signs and symptoms and report any changes or concerns to the physician promptly.
- Educate patients and their families on proper healthcare strategies, disease prevention, and medication management.
- Collaborate with other healthcare professionals, such as RNs and therapists, to provide holistic care to patients.
- Coordinate and communicate effectively with the interdisciplinary team, physicians, and other healthcare providers.
- Document all patient care activities accurately and timely in electronic health records.
- Provide emotional support and encouragement to patients and their families.
Qualifications & Requirements:
- Graduate of an accredited school of nursing with a valid and current Licensed Vocational Nurse (LVN) license by the Texas State Board of Nurse Examiners.
- Preferred minimum of 1 year of clinical nursing experience, preferably in home health or a related field with a minimum of 2 hours of general, critical care, or emergency hospital experience.
- Strong clinical assessment and critical thinking skills.
- Skills and knowledge with infusion therapy and wound care is a bonus but not required.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficient in using electronic health records and other healthcare software.
- Compassionate, empathetic, and patient-centered approach to care.
- Reliable transportation and valid driver's license.
- Basic Life Support (BLS) certification.
