Job Summary

We are seeking a skilled Behavioral Health Case Manager to join our team at Adelante Healthcare. As a key member of our healthcare team, you will be responsible for managing a caseload of clients with long-term mental illness, assessing client needs, developing and implementing service plans, and collaborating with internal and external resources to meet client service needs.

Responsibilities
  • Participate in the screening of new service requests to ensure that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and procedures.
  • Accept newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist.
  • Complete case management assessments on all clients entering caseload and develop appropriate treatment/service plans with clients.
  • Provide direct client services to individuals and/or family members, including case management, skills training, group counseling, and health promotion.
  • Collaborate with other healthcare professionals to ensure that clients receive comprehensive and coordinated care.
  • Document all service contacts on a timely basis and maintain accurate and up-to-date case records.
Requirements
  • Bachelor's degree in Social Service field or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelor's Degree.
  • High school diploma or GED from an accredited institution is required.
  • Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferred.
  • Excellent customer service skills and ability to work effectively with diverse populations.
  • Knowledge of Arizona Department of Health Services and RBHA requirements.
  • Ability to complete a Behavioral Health Assessment with appropriate clinical oversight.
  • Valid Arizona driver's license and Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety.
