Job Summary:

The Behavioral Health Case Manager is responsible for managing an assigned caseload of clients with long-term mental illness. This includes assessing client needs, developing and implementing service plans, and working with internal and external resources to meet client service needs.

Key Responsibilities:
  • Participate in the screening of new service requests and ensure that all individuals are adequately and appropriately served.
  • Develop and implement treatment/service plans with clients to meet their individual needs.
  • Provide direct client services, including case management, skills training, group counseling, and crisis intervention.
  • Collaborate with other internal and external resources to ensure comprehensive care for clients.
  • Document all service contacts and maintain accurate case records.
Requirements:
  • Bachelor's degree in Social Service field or equivalent experience.
  • Minimum of one year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients.
  • Excellent customer service and communication skills.
  • Knowledge of Arizona Department of Health Services and RBHA requirements.
  • Ability to complete a Behavioral Health Assessment with clinical oversight.
