Job Summary

The Case Manager I/II Emergency Department at Children's National Hospital plays a vital role in ensuring the highest quality care for our patients. As a seasoned RN with a strong understanding of emergency care, disease processes, and payer requirements, you will be responsible for taking proactive steps to ensure appropriate ED utilization, place patients in the correct status, and meet patient care coordination needs.

This is a unique opportunity to join a team of experts in pediatric emergency care, working collaboratively with physician practices and the care management team to develop and facilitate effective, efficient care delivery options for patients across the continuum of care.

Key Responsibilities:
  • Conduct comprehensive assessments and develop formal Case Management care plans
  • Establish goals and interventions, and monitor/tracking patient progress
  • Partner with physician practices and the care management team to address clinical and care coordination needs
  • Work with payers and community resources to develop and facilitate effective care delivery options
Requirements:
  • Minimum 5 years of related experience
  • Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing with a Bachelor's Degree in a related field
