Position Overview:
The Patient Care Navigator collaborates with Sutter Health patients from specialized populations (such as oncology, bariatric, spinal trauma, and stroke) to facilitate their journey through a wide range of health services. This role adheres to established policies, procedures, protocols, and guidelines while identifying and addressing barriers to care. The navigator assists with access challenges, fosters relationships with healthcare providers, clarifies insurance and payment systems, and coordinates services among providers, patients, and caregivers within Sutter Health, external facilities, and the community to ensure seamless and integrated quality care. Additionally, the navigator raises awareness about healthcare organizations, agency resources, Sutter Health support programs, community resources, and clinical trials, while addressing other illness-related needs identified by the patient, their family, or support group. The role also involves contributing to the ongoing evaluation and quality enhancement of the program, including community outreach and marketing efforts.

Job Description:

EDUCATION:

  • Equivalent experience may be accepted in lieu of the required degree or diploma.
  • Bachelor's degree in Social Work, Health Education, or a related field, or equivalent education/experience.
  • OR: Graduate of an accredited nursing program.
CERTIFICATION & LICENSURE:
  • RN - Registered Nurse of California.
  • OR LCSW - Licensed Clinical Social Worker.
  • OR RCP - Respiratory Care Practitioner.
  • OR PT - Physical Therapist within one year of hire.
  • OR OT - Occupational Therapist.
  • BLS - Basic Life Support Healthcare Provider certification.
  • Depending on focus area, current breast patient navigator certification or attainment of certification within the first calendar year of employment.
TYPICAL EXPERIENCE:
  • A minimum of 5 years of recent relevant experience.
SKILLS AND KNOWLEDGE:
Proven competency in applying evidence-based knowledge, theories, practices, principles, and processes of professional nursing.

In-depth understanding of the focus area, including diagnoses, current treatment options, complementary therapies, potential complications, family/support group dynamics, and associated quality of life issues.

Familiarity with community resources relevant to the assigned focus area, including requirements and accessibility.

Comprehensive knowledge of healthcare systems, insurance/payment systems, and medical terminology.

Strong analytical and project management skills, including the ability to analyze problems, draw practical conclusions, and implement effective changes.

Excellent written and verbal communication skills to convey clinical information and program data clearly and professionally to diverse audiences.

Proficiency in Microsoft Office Suite, familiarity with databases, and experience with electronic health record systems.

Ability to work independently as well as collaboratively within a team, demonstrating exceptional attention to detail and organizational skills.

Capability to prioritize and delegate tasks under varied and urgent conditions while adhering to standardized procedures and practices to achieve objectives and meet deadlines.

Skill in identifying and resolving complex problems based on the analysis of multiple information sources while providing innovative perspectives on existing solutions.

Commitment to ensuring the privacy of each patient's protected health information (PHI).

Ability to build collaborative relationships with other healthcare providers (physicians, nurses, etc.), healthcare organizations, agencies, and community resources to coordinate efforts and deliver the highest quality of patient care.

Pay Range is $44.28 to $66.42 / hour.

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
