Community Health Nursing Home Care Coordinator Job

We are seeking a skilled Community Health Nursing Home Care Coordinator to join our team at Community Health Centers of the Rutland Region. As a key member of our healthcare team, you will be responsible for coordinating patient care and supporting our nursing home partners.

About the Role:

  • Assist providers in managing patient care workflows.
  • Communicate with SNF nurses regarding new orders and updates.
  • Field calls from nursing staff, address concerns, and escalate complex cases to providers.
  • Update and maintain accurate patient records.
  • Manage regulatory visits and track patient data.

Requirements and Qualifications:

  • Vermont LPN license and valid CPR certification.
  • Prior experience working in nursing homes or similar settings is preferred.
  • Strong technical skills and proficiency in electronic medical records.
