Job Title: Community Health Worker

At Heartland Community Health Center, we are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in providing short-term care coordination and connecting patients to resources that improve their health and overall well-being.

Job Summary

The Community Health Worker will collaborate with our integrated medical team and community-based social services agencies to provide patient-centered care. This role requires strong interpersonal and communication skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities
  • Assist patients in the community or clinic setting, communicating the purposes of our program and its impact on their well-being.
  • Help patients identify socio-economic issues affecting their health and develop health/social management plans and goals.
  • Document patient encounters, contracts, and monthly reports, maintaining comprehensive electronic client files.
  • Provide education on the proper use of the Emergency Room and alternatives, coaching patients on effective management of chronic health conditions and self-care.
  • Assist patients in accessing health-related services, including obtaining a medical home and overcoming barriers to care.
  • Provide support and advocacy during initial medical visits and follow up with patients and providers regarding health/social services plans.
  • Facilitate communication and coordinate services between providers and patients, coordinating and monitoring services to ensure compliance with care plan objectives.
  • Participate in care coordination, provide referrals, and follow up with patients and providers.
Requirements
  • High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
  • Bi-lingual in Spanish and English preferred, but not required.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
  • Knowledge of community agencies and resources, as well as multi-system outreach programs related to healthcare delivery.
  • Ability to plan, implement, and evaluate individual patient care plans.
  • Ability to communicate medical information to healthcare professionals and care coordinators over the telephone.
Working Conditions
  • This role requires movement throughout the building and multiple locations during the day.
  • This role may be exposed to infections and contagious diseases.
  • This role is subject to interruptions and requires management of personnel and procedures in a high-stress environment.
Physical Requirements
  • Moves equipment weighing up to 50 pounds.
  • Moves throughout the building to meet with employees, patients, or visitors.
  • Must be able to remain in a stationary position during shift.
  • Ability to read, write, and speak English.
Details
  • Schedule: Full-time exempt.
  • Hours: Mon-Friday 8:00AM-5:00PM (average 40 hours/week).
  • Location: All Heartland Community Health Center Locations.
