We are seeking a skilled Registered Nurse to join our team in La Crosse and the surrounding area. As a RN Visit Nurse, you will have the opportunity to provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.
Key Responsibilities- Provide clinical nursing assessments and develop individualized plans of care in accordance with federal and state program requirements.
- Ensure patient eligibility and medical necessity for services, and initiate plan of care revisions as needed with physician approval.
- Collaborate with other community providers to develop and revise individualized plans of care and service plans.
- Ensure plan of care frequency and duration meets the needs of the patient.
- A minimum of one year experience as an RN is preferred.
- Current RN licensure in the state of practice is required.
- Current CPR certification is required.
