Key Responsibilities:
- Conduct thorough evaluations to determine the individual needs of each child.
- Serve as the primary support for family reunification initiatives, focusing on identifying relatives both domestically and internationally, and aiding in securing legal guardianship when necessary.
- Design, execute, and assess personalized case management strategies.
- Work collaboratively with multidisciplinary teams to ensure coordinated care and services.
- Maintain comprehensive records of case activities, progress, and outcomes.
