Position Overview:

We are currently seeking a dedicated and compassionate Full-Time Hospice Nurse to deliver personalized and dignified care to our patients. This role is essential in ensuring that individuals receive the support they need in a respectful and loving environment.

Key Responsibilities:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager.
  • Serve as the primary point of contact between the physician, patient/family, and hospice team.
  • Deliver direct patient care services to hospice patients while providing supportive assistance to their families.
  • Educate caregivers on how to care for their loved ones, empowering them in their roles.
  • Collaborate with an interdisciplinary team to ensure comfort, peace, and dignity for patients.
  • Assess patient and family needs, assisting in the development and implementation of individualized care plans.
  • Maintain accurate and up-to-date clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives within the organization.

Qualifications:

  • Active RN license in the state of employment (or eligibility to obtain).
  • A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology settings.
  • Valid driver's license and automobile insurance.
  • Current CPR certification.
