Position Overview:

We are currently seeking a dedicated and compassionate Full-Time Hospice Nurse to provide individualized care and support to our patients. This role is vital in ensuring that our patients receive the dignified and loving care they deserve.

Key Responsibilities:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager.
  • Serve as the primary communication link between the physician, patient/family, and the hospice care team.
  • Deliver direct nursing care to hospice patients while also providing support to their families.
  • Educate caregivers on effective care practices for their loved ones.
  • Collaborate with an interdisciplinary team to promote comfort, peace, and dignity for patients.
  • Assess patient and family needs and assist in developing and implementing care plans.
  • Maintain accurate and up-to-date clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives within the organization.

Qualifications:

  • Active RN license in the state of employment (or eligibility to obtain).
  • A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology settings.
  • Valid driver's license and automobile insurance.
  • Current CPR certification.
