Home Health RN Position

We are seeking a skilled and compassionate Registered Nurse to join our Home Health Care team at Providence Benedictine Nursing Center. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Responsibilities:- Assess patient needs and develop individualized care plans to meet their unique requirements.
- Provide direct patient care, including wound care, medication management, and education on disease management and self-care.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
- Conduct regular patient evaluations to monitor progress and adjust care plans as needed.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Current Oregon Registered Nurse License.
- 1 year of Medical/Surgical experience in an acute care setting.
- 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment.
- Recent Home Health experience preferred.
- Hospice or Palliative Care experience for Palliative Care RN work assignment.
- Point of Care experience.
