Home Health RN Job Description

We are seeking a skilled and compassionate Registered Nurse to join our Home Health Care team at Providence Benedictine Nursing Center. As a Home Health RN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:
  • Provide direct and indirect quality care for patients of all ages, according to individualized needs and the nursing process.
  • Coordinate patient care activities with other members of the healthcare team in a collegial relationship.
  • Work collaboratively with physicians, therapists, and other healthcare professionals to develop and implement patient care plans.
  • Conduct thorough assessments, develop and implement care plans, and evaluate patient outcomes.
  • Provide education and support to patients and their families to promote health and wellness.
Requirements:
  • Current Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • 1 year of Medical/surgical experience in an acute care setting.
  • 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment.
Preferred Qualifications:
  • Recent Home Health experience.
  • Hospice or Palliative Care experience for Palliative Care RN work assignment.
  • Point of Care experience.
