Work Schedule:
40 hours per week; Monday to Sunday, with a varied schedule to meet patient needs, including some evenings.

Role Overview:
Provide targeted, medically necessary services and support to children experiencing severe emotional disturbances, their families, and patients of the Certified Community Behavioral Health Clinic (CCBHC).

Key Responsibilities:
  • Assist families and treatment teams in ensuring children remain in the least restrictive and least intrusive environments.
  • Advocate for children and families as required.
  • Coordinate Community-Based Services (CBS) for youth referred for Integrated Care Services:
    • Engage with patients, caregivers, and treatment team members, either in person or through electronic means, to conduct strengths assessments, discuss treatment objectives, and formulate and execute treatment plans.
    • Support the development and implementation of crisis intervention plans.
    • Ensure assigned patients receive medically necessary services.
    • Provide transportation in a personal vehicle to facilitate patient attendance at medical appointments and other therapeutic activities, including community resource development, employment or education access, and social or consumer-run activities.
    • Complete referrals to contracted providers for services outlined in the patient's treatment plan.
    • Collaborate with COMCARE Medical Records to ensure timely submission of required items to contracted providers.
    • Maintain weekly communication with contracted service providers to ensure patients have initiated the intake processes and assist in overcoming barriers to service provision.
    • Act as a liaison between contracted service providers and assigned treatment providers.
    • Monitor waitlists to ensure contracted service providers meet timely access expectations as stipulated in their contracts.
    • Provide Care Coordination with community providers.
    • Deliver Direct Care to patients, including medically necessary services to patients and their caregivers.
    • Facilitate Specialized Psychosocial Group treatment for high-risk populations in community, school, or placement settings.
    • Utilize Evidence-Based Practice (EBP) techniques, such as Motivational Interviewing, to enhance patient treatment outcomes.

Minimum Qualifications:
Two years of experience in a Community Mental Health Center setting.

A Bachelor's degree or equivalent experience in a human services field, with one year of experience substituting for one year of education.

This position requires a valid US driver's license without restrictions and current proof of automobile insurance as per Sedgwick County policy.

Access to a personal vehicle is necessary to fulfill the driving responsibilities of the role.

Must meet specifications outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.

Completion of orientations provided by Sedgwick County and COMCARE is required.
