As a key member of our team, you will play a vital role in supporting children and families in achieving permanency through reintegration, adoption, custodianship/guardianship, or aged out youth.
Key Responsibilities- Provide support and services to children and families through crisis intervention, risk management, and coordination of community/educational services.
- Meet with children and families as determined by the aftercare plan to support reunification and address or resolve concerns.
- Work closely with public and private agencies providing supportive services to ensure comprehensive support for families.
- Maintain accurate and timely documentation/reports to track progress and ensure accountability.
