Duties and Responsibilities:

As a Home Health Supervisor, you will be responsible for:

  • Receiving case referrals and reviewing available patient information
  • Assigning appropriate hospice/home health personnel to cases as needed
  • Conferencing with the Director regarding individual eligibility for services
  • Reviewing and evaluating each case through home visits, conferences, record review, and services provided by clinicians
  • Discussing and verifying impressions, instructing and guiding clinicians to promote more effective performance and delivery of quality hospice/home health services
  • Assisting clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care
  • Attending case conference meetings with hospice/home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement
  • Assisting in the screening and interviewing process of new employees and making recommendations for employment
  • Assisting in the orientation of new employees
  • Participating in public relation and community activities that promote the organization's role as an effective member of the hospice/home health delivery system
  • Promoting customer service orientation to all hospice/home health personnel
  • Participating actively in quality management programs

Requirements:

To be successful in this role, you will need:

  • A degree in Nursing from an approved school of nursing as determined by the Board of Nursing
  • Licensure to practice registered nursing in the state by the Board of Nursing
  • Recent experience in home health care settings
  • A strong working knowledge of home health regulatory requirements
  • Strong computer skills
  • Strong leadership skills
