Home Health LPN Position

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Epworth Villa. As a Home Health LPN, you will provide direct patient care under the supervision of a registered nurse, focusing on delivering high-quality care to patients in their homes.

Key Responsibilities:

  • Follow the plan of care and provide treatments as defined in the state Nurse Practice Act.
  • Implement the plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participate in coordination of home health services and report identified needs for other services to the registered nurse and/or Clinical Supervisor.

Requirements:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience.
  • Hospice, Home Health, or medical/surgical experience is preferred.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, technical, and organization skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Certificates, Licenses, Registrations:

  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Possess and maintain valid CPR certification.
