Job Title: Home Health LPN

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team at Epworth Villa. As a key member of our team, you will provide direct patient care under the supervision of a registered nurse, ensuring the highest level of care and comfort for our patients.

Responsibilities:
  • Provide direct patient care as defined in the state Nurse Practice Act.
  • Implement the plan of care initiated by the registered nurse.
  • Document patient information accurately and timely, consistent with the plan of care.
  • Assess and educate patients and their families/caregivers on self-care techniques and information relevant to their diagnosis and plan of care.
  • Participate in the coordination of home health services, reporting identified needs for other services to the registered nurse and/or Clinical Supervisor.
  • Utilize equipment and supplies effectively and efficiently, adhering to aseptic technique.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the registered nurse.
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience, with a preference for hospice, home health, or medical/surgical experience.
  • Compliance with accepted professional standards and practice.
  • Excellent verbal and written communication, technical, and organizational skills.
  • Licensed driver with a reliable vehicle and insurance meeting the organization's requirements.
Certifications and Licenses:
  • Current licensure as a licensed practical nurse or licensed vocational nurse in the state.
  • Valid CPR certification.
