You will travel to various patient homes throughout the day, ensuring optimal care and aiding in the transition from hospital to home. Your role will emphasize patient education, promoting independence, and ensuring a smooth recovery process. You will typically work varied hours, allowing for a balance of clinical care and patient interaction. No in-hospital duties.

Responsibilities:

Provide high-quality medical care to patients in their homes
Conduct wound care and post-operative follow-ups
Develop and implement individualized care plans
Educate patients and their families on disease management and self-care
Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

Nursing degree from an accredited school
Oklahoma RN licensure
Valid driver's license
CPR certification
At least one year of Home Health/Hospice experience as a RN
