LPN Primary Care Specialist

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Primary Care team at Ochsner Health. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Responsibilities- Provide direct care to assigned patients, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with the multidisciplinary team to develop and implement individualized patient care plans.
- Assist with patient education and support, promoting health literacy and self-management skills.
- Participate in quality improvement initiatives, contributing to the enhancement of patient care and outcomes.
- Communicate effectively with patients, families, and healthcare team members, ensuring seamless care coordination.
- Current LPN licensure in the state of practice.
- Basic Life Support (BLS) certification from the American Heart Association.
- IV therapy credentials (preferred).
- Excellent organizational and time management skills, with the ability to work in a fast-paced environment.
- Good interpersonal and delegation skills, with the ability to work effectively with patients, families, and healthcare team members.
