We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to pediatric patients in their homes. As a Home Care LPN, you will work under the supervision of a Registered Nurse and be responsible for providing direct nursing care, recording clinical notes, and coordinating patient care activities.
Responsibilities- Provide high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice.
- Document patient visits thoroughly and completely in the electronic medical record.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
- Ensure confidentiality of patient health information.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
