We are seeking a highly skilled and experienced Clinical Manager RN to join our team at CenterWell Home Health. As a Clinical Manager RN, you will be responsible for overseeing the delivery of high-quality patient care services in a home health setting.
Key Responsibilities- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct assessments and reassessments of patients, including updating care plans and interpreting patient needs.
- Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location.
- Ensure patients meet admission criteria and make decisions to admit patients to service.
- Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing patient Plan of Care (POC).
- Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers to ensure the appropriateness of care and outcome planning.
- Work in conjunction with the Branch Director and Company Finance Department to establish location's revenue and budget goals.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees assigned to a specific location and carry out supervisory responsibilities in accordance with Company policies and procedures.
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
- Home Health experience highly preferred.
- Management and people leadership experience required.
- OASIS experience highly preferred.
- Homecare Homebase (HCHB) experience preferred.
- CMS PDGEM billing experience preferred.
- Experience managing and leading multidisciplinary teams of 15+ employees highly preferred.
- Strong background in home health clinical operations and quality assurance highly preferred.
- Experience supporting home care program initiatives around cardio, ortho, wound care, safe strides, and various therapy initiatives highly preferred.
