We are seeking a highly skilled and compassionate Hemophilia Triage Nurse to join our team at the Indiana Hemophilia and Thrombosis Center, Inc. This is a unique opportunity to work with a multidisciplinary team to provide exceptional care to patients with bleeding disorders.
Key Responsibilities- Triage and Assessment: Conduct thorough telephone assessments of patients with bleeding disorders, including hemophilia, and develop individualized care plans to ensure optimal treatment and outcomes.
- Clinical Expertise: Provide expert clinical guidance and support to patients, families, and healthcare providers on bleeding disorder management, treatment options, and disease progression.
- Communication and Education: Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and education on bleeding disorder management.
- Documentation and Reporting: Accurately document patient encounters and outcomes in the electronic medical record system, and provide regular reports to the healthcare team on patient progress and treatment effectiveness.
- Education: Completion of an approved nursing program.
- Experience: Minimum 2 years of experience in a related field, preferably in hemophilia care.
- Licensure: Valid RN license in the state of Indiana.
- Other: Valid Indiana Driver's license and automobile insurance.
