We are seeking a skilled Nurse Triage Specialist to join our team at the Indiana Hemophilia and Thrombosis Center. As a key member of our multidisciplinary staff, you will be responsible for triaging incoming telephone calls and providing high-quality nursing care to patients.
Key Responsibilities- Conduct thorough telephone nursing assessments and develop individualized nursing care plans
- Advocate for patient/family needs and monitor for healthcare-related outcomes
- Communicate lab and testing results to patients/families
- Manage prescription refills under the direction of a physician
- Document encounters in the electronic medical record (EMR) system
- Completion of an approved nursing program
- Minimum 2 years of related experience
- Valid RN license in the state of Indiana
- Valid Indiana Driver's license and automobile insurance
