We are seeking a highly motivated Patient Care Coordinator Specialist to join our team at HEALTH CONNECT AMERICA, INC.
About the Role
This position plays a critical role in supporting our care coordination and management services. As a Patient Care Coordinator Specialist, you will be responsible for performing general outreach, engagement, and follow-up with members, coordinating services and appointments, engaging in health promotion activities, and providing referrals and resources to support members' needs.
Key Responsibilities:
- Perform daily outreach and engagement activities to ensure timely communication with members.
- Coordinate services and appointments, including scheduling reminders and arranging transportation as needed.
- Engage in health promotion activities, such as knowledge sharing and education, to support members' overall well-being.
- Provide referrals and connect members with community-based resources and social support services to address their unmet needs.
- Participate in case conferences to collaborate with the care team on member care plans.
- Support the care manager in assessing and addressing unmet health-related resource needs.
Requirements:
- You must be at least 18 years old.
- You should have a high school diploma or equivalent.
- You must meet one of the following requirements:
- You are a person with lived experience with an intellectual or developmental disability (I/DD) or a traumatic brain injury (TBI) with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.
- You are a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist.
- You are a parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and have at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member).
- You have two years of paid experience performing functions similar to those described above.
What We Offer:
