LifeBridge Health seeks a compassionate patient care specialist to join its team. This role requires providing direct services to patients and families, identifying and resolving problems that impede discharge planning or the use of healthcare resources.

This position involves contact with patients, families, physicians, nursing staff, and internal/external resources. Effective documentation of all interventions and services provided is essential.

The ideal candidate will possess strong assessment skills, enabling them to provide discharge planning for adult and geriatric populations. They will also demonstrate knowledge of growth and development principles across the lifespan, interpreting relevant information to identify each patient's needs.

In this role, you will intervene with patients and families regarding emotional, social, and financial consequences of illness and/or disability. You will work directly with patients exhibiting complex family dynamics impacting patient care.

You will determine appropriate psychosocial interventions involving abuse/neglect cases, domestic violence, guardianship, and mental health issues. Additionally, you will access and mobilize family/community resources to meet identified needs.

This position collaborates with physicians, nursing staff, and other multidisciplinary team members to promote care coordination, attending daily rounds.

Able to maintain timely and consistent documentation, prepare care plans for forensic patient court hearings, and have a Master's Degree in Social Work/Counseling from an accredited institution. Current licensure in Maryland (LMSW/LGPC) is required, with LCSW-C/LCPC preferred.
