Job Description

The Patient Care Coordinator RN will play a vital role in ensuring seamless patient transitions throughout the continuum of care. This dynamic individual will be responsible for coordinating interdisciplinary plans of care, advocating for patients' needs, and maintaining ongoing communication with care teams.


Required Skills and Qualifications

To be successful in this role, you will need:

  • A valid Registered Nurse (RN) licensure in Maryland
  • BLS certification from the American Heart Association or American Red Cross
  • Case management certification from the Commission for Case Manager Certification (CCMC) preferred
  • Diploma from an accredited school of nursing or required professional licensure at hire
  • Basic understanding of Medicare and Health Insurance
  • Experience working with diverse communities
  • Knowledge of EMRs, such as Meditech, eCW, Point Click Care, and OCCP
