Client Support Specialist

We are seeking a dedicated and compassionate Client Support Specialist to join our Psychiatric Rehabilitation team at Crossroads Community, Inc.
This is a full-time position that offers a competitive salary of $55,000 per year, paid 100% by CCI, as well as excellent benefits, including health insurance, 403(b) plan, and educational assistance opportunities.
Job Responsibilities- Provide person-centered support in a team environment to clients on-site at the program and in the community.
- Assess consumers' needs and strengths, assist with development of the rehabilitation plan, assess progress, and maintain the clients' medical record.
- Coordinate services provided by Crossroads for assigned clients, including transportation and daily documentation requirements.
- High School Diploma or equivalent.
- Credit given to AA, BS in Behavioral Health, Psychology, Social Work, or related field and 2 years of positively referenced work history in human services field.
- Professionalism, patience, and strong communication skills.
