Role Overview: The role of a Care Coordinator is pivotal in assisting individuals to navigate the complexities of the healthcare system. Many clients face challenges in accessing care and advocating for their needs. Our Care Coordinators engage with clients regularly, collaborating with them and their providers to optimize their treatment experience.

Work Schedule: This position operates within Central Indiana, with flexible hours available from Monday to Friday, either 8 AM to 4 PM or 9 AM to 5 PM.

Key Responsibilities:

  • Act as the primary liaison for clients on your caseload, facilitating improved access to integrated healthcare and advocating for their needs.
  • Utilize creativity and flexibility to identify barriers to care, assist clients with transportation solutions, employ motivational interviewing techniques to enhance engagement, and manage appointment scheduling.
  • Assess client satisfaction and advocate for their preferences during treatment, encouraging self-advocacy.
  • Coordinate communication among clients' various healthcare providers through regular contact, including meetings and correspondence.
  • Conduct routine chart reviews to ensure treatment plans align with the supervising psychologist's directives and client preferences.

Qualifications:

  • Bachelor's degree in behavioral health or a related field.
  • Valid driver's license with a satisfactory driving record and access to insured transportation.
  • Familiarity with Medicaid/Medicare guidelines, Marketplace plans, and commercial insurance benefits is preferred.
