Job Summary

We are seeking a highly skilled Oncology Nurse Navigator to join our team at American Oncology Network, Inc. This is a critical role that requires a strong clinical background, excellent communication skills, and a passion for delivering exceptional patient care.

Key Responsibilities
  • Collaborate with healthcare providers to coordinate patient care and ensure seamless transitions between treatment phases.
  • Provide education and resources to patients, families, and caregivers to facilitate informed decision-making and timely access to quality health and psychosocial care.
  • Assist patients with cancer, families, and caregivers to overcome healthcare system barriers and access necessary services.
  • Coordinate extension of services to include nutrition, home health, DME, and hospice care.
  • Develop and implement cancer survivorship plans to support patients' long-term health and well-being.
Performance Expectations
  • Meet or exceed performance metrics for patient satisfaction, clinical trial participation, and navigation program outcomes.
  • Develop and maintain strong relationships with healthcare providers, community navigators, and other stakeholders to ensure effective communication and collaboration.
  • Participate in quality improvement initiatives and contribute to the development of best practices in oncology navigation.
Requirements
  • Minimum of 5 years' experience in oncology nursing, with a strong clinical background and expertise in cancer care.
  • Previous experience in clinical trial coordination and management is highly desirable.
  • Strong analytical and problem-solving skills, with the ability to exercise independent judgment and make sound decisions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders and populations.
  • Proficiency in MS Office Word, Excel, PowerPoint, and Outlook, with experience in electronic health records and other relevant software systems.
Preferred Qualifications
  • OCN certification and/or other relevant certifications in oncology nursing.
  • Public Notary certification and/or other relevant certifications in patient advocacy.
  • Valid driver's license and ability to travel to satellite offices and offsite meetings.
