Position Summary

Guaranteed Health is looking for dedicated and empathetic Hospice Registered Nurses to join our expanding Hospice Care Teams. Our nurses understand the significance of hospice care in relieving pain and suffering, aiming to support patients in achieving a dignified end-of-life experience. You will be responsible for planning, organizing, and directing hospice care with a focus on community health education.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to identify hospice needs.
  • Develop individualized care plans that set goals based on nursing diagnoses, involving patients and families in the planning process.
  • Implement preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.
  • Assist patients and families with healthcare instructions and daily living activities to promote self-sufficiency and optimal comfort.
  • Maintain accurate clinical documentation regarding patient conditions and care provided, coordinating with community health professionals.
  • Participate in interdisciplinary meetings and on-call duties as necessary.
  • Ensure the availability of necessary equipment and services, supervising ancillary personnel as required.
  • Engage in quality improvement initiatives and complete virtual visits as needed.
Essential Skills and Qualifications
  • Expertise in caring for terminally ill patients, including medication administration and emotional support.
  • Strong interpersonal skills and a compassionate demeanor.
  • Ability to work independently with minimal supervision.
  • Experience in educating patients and caregivers on self-care techniques and medication management.
  • Background in providing continuity of care with an interdisciplinary team to address the physical, spiritual, and emotional needs of patients and families.
  • Excellent communication skills, both verbal and written, along with strong problem-solving abilities.
  • Commitment to personal and professional growth through ongoing education and training.
Qualifications
  • Bachelor of Science in Nursing (BSN) with a current RN license and CPR certification.
  • A minimum of two years of nursing experience, with at least one year in home care or hospice settings.
  • Must possess a valid driver's license and have access to a reliable vehicle.
  • Physical requirements include the ability to stand, walk, lift, and transfer patients, as well as perform various physical tasks.
  • Experience with virtual patient visits is a plus.
