Key Responsibilities- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to ensure comprehensive care and support.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans, medications, and other important information.
- Participate in ongoing education and training to stay current on best practices, new technologies, and emerging trends in hospice care.
- Graduate of a Vocational Nursing program.
- California Licensed Vocational Nurse upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- Phlebotomy/IV Certification required, must obtain within 6 months of hire.
- 1 year of nursing experience.
- Community health/hospice/palliative care or medical/surgical experience.
