About the Role

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our Hospice team at Providence. As a LVN, you will play a vital role in delivering high-quality patient care and providing emotional support to individuals and their families.

Key Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Collaborate with healthcare professionals to ensure seamless care transitions and coordination.
  • Document patient progress, including medical history, treatment plans, and outcomes.
Requirements
  • Graduate of a Vocational Nursing program.
  • California Licensed Vocational Nurse upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Phlebotomy/IV Certification required. Must obtain within 6 months of hire.
  • 1 year - Nursing experience.
Preferred Qualifications
  • Community health/hospice/palliative care or medical/surgical experience.
