About the Role

We are seeking a skilled Registered Nurse to join our PACE/Elderplace team as a Care Manager. In this role, you will provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Coordinate with healthcare providers, family members, and caregivers to ensure seamless care delivery
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices
  • Collaborate with the interdisciplinary team to ensure comprehensive care and address any concerns or issues that may arise
  • Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed
Requirements
  • Current Oregon Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
  • 1 year of experience in a PACE organization, with a focus on care management and coordination
