RN - PACE/Elderplace Care Manager

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Elderplace team as a PACE/Elderplace Care Manager. In this role, you will provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.
Key Responsibilities- Provide care management and care coordination services to patients and their families
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure comprehensive care and services
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families on disease management and self-care
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager upon hire
- 1 year of experience in a PACE organization
