About the Role

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Holy Name Medical Center. As a Patient Care Coordinator, you will play a vital role in providing exceptional patient care and support to our patients and their families.

Key Responsibilities

  • Provide basic patient care procedures under the direct supervision of a Registered Nurse, including vital sign measurements, intake and output monitoring, and personal hygiene care.
  • Assist patients with their meals and feed patients who are unable to feed themselves, ensuring their nutritional needs are met.
  • Obtain routine specimens of urine, sputum, and feces, and perform treatments as directed by the RN, such as enemas, ice collar, sitz baths, and perineal care.
  • Perform CPR and assist with post-mortem care, demonstrating a commitment to patient safety and well-being.
  • Assist patients with mobility, including getting in and out of bed, walking, and positioning, using safe lift equipment as needed.
  • Clean and organize bedside units, utility rooms, nourishment areas, and nurses' stations, maintaining a clean and safe environment for patients and staff.
  • Perform other duties as assigned, demonstrating flexibility and a willingness to adapt to changing priorities.

Requirements

  • High School Diploma or equivalent required.
  • 1 year of relevant or related experience preferred.
  • Knowledge of basic medical terminology, strong interpersonal and team skills, and ability to work under pressure.
  • HEARTSAVER CPR/AED certification required.
