Responsibilities:- Conduct comprehensive assessments of patients' social, psychological, and environmental situations to identify their needs and challenges.

- Connect patients and families with appropriate community resources and support services, such as financial assistance programs, counseling centers, and rehabilitation facilities.

- Bachelor's degree in Social Work (BSW) is required; Master's degree in Social Work (MSW) is preferred.
- Previous experience working in a hospital or healthcare setting is highly desirable.
- Current and valid New Jersey social work license.
- Excellent communication, interpersonal, and problem-solving skills.
- Strong commitment to patient advocacy and ethical practice.
