Key Responsibilities- Oversee the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for the Minimum Data Set (MDS) completion and electronic state submissions.
- Contribute to the development of personalized resident Care Plans, assessing individual needs and anticipated outcomes in accordance with federal and state regulations.
- Provide training to new interdisciplinary team members on clinical content and the electronic completion of MDS items.

To be considered for this role, candidates must meet the following criteria:
- Current LPN license in good standing.
- Previous experience in a healthcare setting, such as a hospital or long-term care facility.
- Knowledge of nursing practices, medical procedures, and regulations governing long-term care.
- Proficiency in computer applications, with AccuMed experience preferred.
- Experience in completing MDS assessments and Care Plans is essential.
- Current BLS/CPR certification is required.
