Safety and Standards
We take pride in adhering to and exceeding CDC and CMS standards. The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing relevant items on the Minimum Data Set (MDS) and associated Care Area Assessments (CAAs) to ensure optimal patient care and appropriate nursing center reimbursement.

Key Responsibilities
  • Oversee the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion, computer encoding, and electronic state transmission.
  • Collaborate in creating individualized resident Care Plans, assessing resident needs and anticipated outcomes in accordance with federal and state regulations.
  • Provide training for new interdisciplinary team members on clinical content and the electronic completion of MDS items.

Qualifications
Applicants must hold a current LPN license in good standing. Previous experience in a hospital, long-term care facility, or similar healthcare environment is essential. Knowledge of general, rehabilitative, and restorative nursing practices, as well as regulations governing long-term care, is required. Proficiency in computer applications, particularly AccuMed, is preferred. Experience as an MDS Coordinator and in completing MDSs and Care Plans is necessary, along with demonstrated expertise in RAI/PPS processes. A current BLS/CPR certification is also required.
