Case Manager, Community Services Coordinator

CAMBA is seeking a highly skilled and empathetic Case Manager to join our team in Brooklyn. As a Case Manager, you will play a vital role in supporting individuals who are experiencing homelessness and require assistance with stabilizing their lives.
Compensation: $48,000 annually
Job SummaryThe Case Manager will be responsible for providing direct services to clients, including intake assessments, service planning, and referrals to community resources. This is a full-time position (35 hours per week) offering a comprehensive benefits package, including health insurance, dental insurance, 403(b) retirement plan with employer match, paid time off, and paid holidays.
Key Responsibilities:
- Conduct intake assessments and develop personalized service plans for clients
- Connect clients with community resources, such as housing, employment, and mental health services
- Monitor client progress and document outcomes
- Escort clients to appointments and assist with applications for benefits and entitlements
- Maintain accurate and up-to-date client records
Requirements:
- Bachelor's or Associate's degree with one year of equivalent experience, or two years of equivalent experience with sufficient education and technical expertise
- Relevant experience working with individuals with histories of homelessness, mental illness, and/or substance abuse
- Excellent written and verbal communication skills
- Proficiency in Microsoft Office Suite
- Bi-lingual English and Spanish preferred
