Job Summary

We are seeking a highly organized and detail-oriented Client Care Coordinator to join our team at Hackensack Meridian Health. As a Client Care Coordinator, you will be responsible for scheduling client visits, tracking visits, and coordinating case conferences.

Key Responsibilities

  • Schedule client visits in accordance with our standardized process, ensuring that only visits ordered by a physician are scheduled.
  • Notify supervisors of any requests that require a physician order and update client schedules daily to ensure they are current and accurate.
  • Verify authorizations and notify clinical supervisors of any activities that require direction or input.
  • Track and maintain disaster plans as needed.
  • Validate caregiver documentation against schedules and reconcile any discrepancies.
  • Recognize patient grievances and direct them to the appropriate manager.
  • Assist the on-call RN with weekend and daily on-call processes.
  • Ensure that all assigned Service Orders have been scheduled in the appropriate system.
  • Answer phone calls promptly and in a courteous manner, taking accurate and complete messages and relaying them to the appropriate party.
  • Maintain client and employee schedules accurately and enter them into the system in a timely manner.
  • Fax transfer forms to the appropriate facilities and coordinators within 24 hours.
  • Perform other duties and projects as assigned, demonstrating flexibility with job responsibilities.

Requirements

  • High School diploma or equivalent required; Associate's Degree preferred.
  • Two years of customer service experience.
  • Proficient in Microsoft Office Suite.
  • Strong organizational skills.

Preferred Qualifications

  • Healthcare experience.
