Job Overview

At Open Door Supported Living Services, we are seeking a highly skilled and compassionate Community Service Facilitator to join our team. As a key member of our support services team, you will play a vital role in ensuring the well-being and independence of our clients.

Key Responsibilities
  • Develop and implement Circle of Support plans to promote client independence and community integration.
  • Coordinate the development and review of Individual Service Plans (ISPs) to ensure client needs are met.
  • Provide training, support, and direct assistance to Direct Support Professionals (DSPs) to enhance their skills and confidence.
  • Conduct regular home visits to monitor client health and safety, and provide emergency assistance as needed.
  • Coordinate the delegation of assistance to clients, ensuring they receive the support they need to thrive.
  • Monitor the effectiveness of services provided to clients and DSPs, and make recommendations for improvement.
  • Assist with the hiring, training, and development of DSPs to ensure a skilled and engaged workforce.
  • Complete required paperwork, including ISP packets, monthly summaries, and performance evaluations.
  • Provide direct personal care to clients as needed, including feeding, toileting, and personal hygiene.
  • Facilitate communication and teamwork among clients, families, and support teams to ensure seamless care.
  • Ability to physically provide for client safety and well-being, including lifting, transferring, and positioning.
  • Assist with scheduling and staff training to ensure a smooth and efficient support process.
  • Schedule client medical appointments and provide transportation as needed.
  • Perform other duties as assigned to ensure the best possible outcomes for our clients.
