About the Role

We are seeking a skilled Hospice Social Worker to join our multidisciplinary team. As a key member of our team, you will be responsible for assessing and providing support and intervention for the social and emotional needs of patients and their families.

Key Responsibilities
  • Conduct psycho-social assessments to identify patient needs and develop effective care plans
  • Provide mental health evaluations and referrals as needed
  • Offer crisis intervention and environmental assistance to patients and families
  • Advocate for patients and families to ensure continuity of care and services
  • Develop and implement after-care plans to support patients and families
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Social Work
  • Two years of recent related work experience, preferably in a hospice care setting
  • Current licensure as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW)
