Job Summary:

The primary role of the Medical Social Worker II is to provide comprehensive social work services to patients and their families/caregivers, addressing the social and emotional aspects of their healthcare experience. As a key member of the healthcare team, the Medical Social Worker II will work under the general direction of the Social Work Manager/LCSW to deliver age-appropriate social work care, ensuring that patients and their families receive the necessary support and resources to navigate their healthcare journey.

Key Responsibilities:
  • Assessment and Intervention: Conduct thorough psychosocial assessments to identify patients' and families' needs, and develop and implement individualized treatment plans to address their social, emotional, and practical concerns.
  • Counseling and Support: Provide counseling services to patients and their families, including crisis intervention, grief and bereavement support, and stress reduction techniques to promote healthy coping strategies.
  • Resource Referral and Coordination: Identify and connect patients and families with community resources, Kaiser Permanente services, and other social work services to meet their complex needs.
  • Collaboration and Communication: Work closely with the healthcare team, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, assessments, and treatment plans, as well as participate in quality improvement initiatives to enhance patient care.
Requirements:
  • Education: Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE)-accredited program.
  • Experience: At least 2 years of social work experience, with a preference for experience in a healthcare setting.
  • Licensure: LCSW licensure is preferred, but not required.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
Preferred Qualifications:
  • Post-MSW experience: At least 1 year of post-MSW experience in a healthcare setting, with a preference for experience in a Kaiser Permanente facility.
  • LCSW licensure: LCSW licensure is preferred, but not required.
