Position Overview:

We are currently seeking a Full-time Hospice Registered Nurse Case Manager to serve patients in various locations. As a vital member of our interdisciplinary team, the RN Case Manager operates under the guidance of the Director of Nursing, the attending physician, and the interdisciplinary group to provide skilled nursing assessments, care planning, and interventions that prioritize the comfort and health of patients and their families.

The responsibilities of this role include:
  • Managing a designated patient caseload as assigned by the Patient Care Coordinator or their representative.
  • Taking primary responsibility for assessing, planning, implementing, and evaluating patient care.
  • Communicating effectively with attending physicians, hospice staff, and other agencies to coordinate comprehensive care.
  • Maintaining ongoing communication with the attending physician regarding patient status and family needs.
  • Collaborating with team members to enhance the patient care plan through shared insights and support.
  • Documenting patient information accurately and promptly in electronic medical records.
  • Contributing relevant information during Interdisciplinary Group meetings.
  • Coordinating physical care by educating primary caregivers and providing direct care as necessary.
  • Engaging with hospice nursing staff to discuss caseload challenges and share professional development opportunities.
  • Facilitating patient care conferences to address issues and evaluate care strategies.
  • Supervising and documenting the activities of nursing assistants as required.
  • Alerting the Patient Care Coordinator to any unusual or concerning patient or family situations.
  • Fulfilling the role of associate nurse during off-hours or when primary nurses are unavailable.
  • Providing compassionate support during the time of death and throughout the bereavement process.
  • Participating in orientation and in-service training programs for staff.
  • Engaging in community programs to enhance professional growth and understanding of hospice care.
  • Staying informed about agency policies and relevant state and federal regulations governing hospice services.
  • Accepting additional responsibilities as needed.
Candidate Qualifications:
  • Current licensure as a registered nurse in the state.
  • A minimum of 1-2 years of nursing experience, with at least one year in hospice care preferred.
  • Proficient in assessing and addressing the needs of patients and families in diverse environments.
  • Demonstrated nursing skills, with the ability to manage emotional stress and respect individual lifestyles.
  • Capable of making informed observations, conducting thorough assessments, and preparing accurate reports.
  • A collaborative team player who supports an interdisciplinary approach to patient care.
  • Possession of a valid driver's license, a clean driving record, and access to reliable transportation.
  • Ability to work independently and as part of a multidisciplinary team in a professional and positive manner.
