Clinical Case Manager

We are seeking a highly motivated and compassionate Registered Nurse to join our hospice care team. As a Hospice RN, you will play a critical role in delivering high-quality patient care and support to individuals with serious illnesses.

Key Responsibilities- Develop and implement individualized care plans for patients, taking into account their unique needs and goals
- Provide direct patient care services, including administering medications, wound care, and other treatments as needed
- Maintain accurate and detailed records of patient interactions, including progress notes and medication lists
- Collaborate with the healthcare team to ensure seamless transitions of care and minimize hospital readmissions
- Educate patients and their families on disease management, symptom control, and end-of-life care
To be considered for this role, candidates must possess:
- A current RN license in the state of employment (or eligibility to obtain)
- At least 1 year of experience in hospice or palliative care (strongly preferred)
- Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
