Aveanna Healthcare is seeking a dedicated and detail-oriented Hospice RN Care Transitions Relationship Manager to join our team.

About the Role

This role serves as a liaison between Aveanna and various healthcare stakeholders, including physicians, clients, hospitals, and medical personnel. The successful candidate will be responsible for evaluating satisfaction of home care services, identifying areas for improvement, and developing effective solutions to meet customer needs.

Key Responsibilities
  • Collaborate with healthcare professionals to assess client satisfaction and identify opportunities for growth.
  • Develop and implement plans to address client concerns and improve service quality.
  • Build strong relationships with hospital discharge teams to ensure seamless transitions of patients to Aveanna services.
  • Provide educational resources to healthcare professionals about Aveanna's services and benefits of home health and hospice care.
  • Identify new referral sources and develop mutually beneficial relationships with healthcare providers.
  • Conduct ongoing needs assessments of customers and respond accordingly or refer them to relevant departments.
  • Participate in enhancing the effectiveness of liaison meetings and serve on agency committees.
  • Assist in creating educational materials and present them to customers.
  • Meet or exceed referral and admission quota expectations for assigned locations.
Requirements
  • RN or LPN licensure required.
  • Minimum of two years' experience in healthcare, home health, or hospice sales.
  • Familiarity with standards of practice for all services provided.
  • Active and unencumbered professional clinical license in assigned territory.
  • Valid driver's license with evidence of automobile liability insurance.
Estimated Salary: $75,000 - $90,000 per year

Aveanna Healthcare offers a competitive salary range based on location and experience. Additionally, employees must comply with all health-related requirements, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.
