Amedisys is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized plans of care.
Responsibilities- Conduct thorough patient assessments and develop comprehensive plans of care in collaboration with the care team.
- Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
- Promote patient health and independence through teaching and rehabilitative measures, assisting patients in learning self-care techniques.
- Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
- Ensure timely and accurate documentation of patient care and progress.
- Regularly communicate patient progress to the clinical manager and care team.
- Develop and implement staff education programs.
- Participate in on-call responsibilities and provide on-call services to patients and families as assigned.
- Engage in ongoing clinical development and continuing education programs.
- One (1+) year of clinical experience as a Registered Nurse (RN).
- Current RN license, specific to the state(s) assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
